Judges receive hundreds of flimsy medical excuses every week. To guarantee your dismissal, your doctor's note must be printed on official clinic letterhead, signed by the physician, and explicitly state that your condition makes serving impossible. It does not need to disclose your specific diagnosis if you prefer privacy, but it must state the functional limitation.
Have your doctor copy and paste this onto their letterhead:
